Check in:
If you are a new patient, please fill out the information on the clipboard in the office lobby or click the link below to bring the paperwork to your session. Make sure you have a food log or list of foods you eat at approximate times in a typical day in writing. Even if you have it memorized and can recite this, it will take away from the time needed for your nutrition session. Thank you.

All payments are due at the time of service. Please see the FAQ Page for rates and fees.

Please note:
The phone will go directly to voicemail if Amy is in session. All calls will be returned within 48 business hours. Please leave your full name, phone number and reason for your call. If you don't get a return call in 48 hours, please try again as she may not have gotten your full phone number, especially if calling from a call phone. Be sure to have the directions prior to your session as your call may not be answered in time if you are lost. Scheduling changes must be made 24 hours prior to your appointment to avoid being charged for your session. Monday appointments must be rescheduled on the Friday prior.
